Having trouble adjusting table columns and rows

My form has a table located within it. I would like the row that reads "Have you ever had an allergic reaction to:

(This would include a severe allergic reaction [e.g., anaphylaxis] that required treatment with epinephrine or that caused you to go to the hospital. It would also include an allergic reaction that occurred within 4 hours that caused hives, swelling, or respiratory distress, including wheezing)"

SDTTo go all the way across without the bullet hole column. I could not figure out how to do that.

    Dear btalley,

Unfortunately, it is not possible to have a separate text box in the selection row of a table.

I believe you should just have N/A checked fields.

    Best,
