Quickly schedule a 2nd appointment

Pharmacies are also looking for a resource to quickly help them schedule 2nd shot vaccinations for Covid-19 vaccines that are typically 21days to 28days after the first shot. We need to schedule these at the time they come in for their first shot. Do you have a way we could easily do this if the JotForm website was open on a laptop while patient is with us to schedule the appointment?

You can use the Appointment element for scheduling appointments. However, you can't perform calculations on an Appointment element so you'd need to schedule the 2nd appointment manually.

The Date element, on the other hand, supports calculation so you can automatically add a number of days to it like in this sample form.
