I am still trying to figure out how to save each application (form) that is completed to look "JUST" like the form they finish with the radio buttons clicked - not an all word doc. I have tried twice and the instruction were not clear and when i wrote back - to ask more questions - i didnt get a response (or didnt see it and i really have been waiting for the answers). I am sure i didnt do something correct becasue it doesn't work. I want each application after it is submitted to be emailed to me (the admin for the project) and it should look just like the finished application (form). Because i have to email that PDF (or some format that won't change) to a committee for approval. It must be exactly like the finished application. Can you help or point me in the right direction?

ERRRRRR i am so sorry BUT i am VERY frustrated. You keep sending the SAME instructions and it does not work (I truly believe i am following the very light instructions correctly - i should be going into the application form correct?). I have gone in and clicked on PDF LINK and i can see the the words "PDF link" listed. However i am having people test the form and i keep getting an email with thier form in all WORDS (not even close to the look of the application)- the light blue color is not there the radio buttons are not there - it is NOT a PDF of the form they turn in (it may include all the words) however i am asking that it looks exactly like the form. I would like to send a screen shot so you know what i mean - however I am not sure where to put it in this email - most of the time when i right back i get an email saying it was "Your last reply came out empty, please resend it using the next link..... It like you can recieve my thank you emials and when i need more help they come back Your last reply was empty...
Please help - i like your form and have invested a TON of time on this and i am on the final lap - please help.

Kay, We do apologize if this has frustrated or upset you. I believe that we have explained it correctly but I think the real problem is that the guide is not helping you to view the form in the exact same way that you want it.
The trouble with this is that the .PDF File compared to the Live Form and the Formats between the two are viewed and saved differently therefore they won't contain the same exact look. Even when you have a form submitted to you it also does not appear the same way nor in your email notification as all of these are designed mostly to quickly show you just the necessary information.
You can upload screenshots with our Image Upload Tool if you wish to show us a screenshot as you mentioned in your last response. Alternatively you could host it with Imgur and do the same thing or even just post a link to your screenshot.
The closest you can get to the Actual Look of the Form is Creating a PDF Version of your Form but then the only way to use it is by turning it into a Dynamic PDF or by using it in a PDF Editor which breaks the submission part to JotForm to my knowledge. However, this is something that we have not created a feature for yet for live usage as such.
