Original PDF Attachment: why does it show a different title on Chrome tab?

When I open the PDF form in Chrome it says 'One Hyde Park - Cable Installations' when opened in Chrome? In the email it shows as 'sickness-self-certification-absence-form' as it should, and when I save it to the computer the suggested name comes up as 'sickness-self-certification-absence-form' too. I'm unsure whether I should be concerned, and would like to ensure that there is no data breach here. Could I also check if this can be resolved / get an explanation of why it is happening ?

Upon checking, the form shows sickness-self-certification-absence-form.
I have cleared your form cache from the server.
Kindly check and let us know if the issue persists.

I have also just tried clearing my cache from the browser – it still says One Hyde Park, when loading it says ‘sickness-self-certification-absence-form’ briefly but then it reverts to One Hyde Park – Cable Installations.

It is certainly something related to your browser cache/cookies. I tested it here, but I can see the right form title - Sickness Self Certification Absence Form, and it did not change at any moment.
I recommend you to clear your browser cache again and cookies if just clearing cache is not enough.

If you have also cleared your browser cookies, this should have worked. Unfortunately, this is an issue related only to your browser. Your clients will see the correct form title.
Can you please test your form again using the Chrome incognito window?
