Thanks for the reply.
You're right, it doesn't seem to be producing the issue in the JotForm preview.
However, I have embedded it in my website at www.webs.com with the correct code and this is the result.
Please tell me if you can see the issue now?

I'm sorry for the trouble caused to you.
When the form is loaded initially, it displays all the question and then hides based on the condition entries you have. Is that the issue you are facing correct?
Upon checking your form, I found that you have several show/hide conditions. There is better way of organizing your conditions with the form collapse tool. You can show/hide multiple questions based on only one condition entry. Here is a guide which should help you : http://www.jotform.com/help/196-How-to-Hide-or-Show-Multiple-Fields-at-Once-Based-On-a-Single-Conditional-Entry
I believe this should solve your problem.
Do get back to us if you have any questions.

Thanks for your reply again.
Yes this is the issue.
I have included form collapse in a test form that I have created here:
and when I load this code into my webpage, the same issue happens (the form is loaded initially). I understand using multiple fields in a single coniditional entry but I am not sure if I can acheive what I am trying to acheive in this form in one entry.
Are you able to show me how I would do that?

Your form works just fine here though like you've said all the field appears before it hides once the conditions are executed. This is very common on the form since it has to be rendered on the browser first, and it requires time specially if that's a huge form with few conditions setup.
Have you tried to embed your form using source code? This might help since the code will be hosted on your end not within Jotforms server so this might give a little improvement on the rendering process. You can also download your form's full source code.
Let us know if this doesn't help. Thank you!
