    Hi,

That form itself is an application that is a little too advanced to be copied accurately by Jotform's Form Builder. The closest thing Jotform can offer is the Purchase Order form, which enables you to list products you want to sell. There is also an option to display the total price of the products selected.

You can find the Purchase Order tool under the Payment Tools section:

Let me know if you have more questions regarding this. Thank you for using Jotform.

Any chance that the Purchase Order Tool can be configured to allow grouping products into categories?

For instance I would like to be able to separate things as follows:

Service options list 1 (hourly rate for service)

Serviceoptions list 2 (hourly rate for service)

Service options list 3 (hourly rate for service)

    Travel fee or Zone fee

How long (this would multiply with the service option)

Alternatively if it were possible to have a group of price options appear based on logic, thatcouldeven work but I'm sure it would not be so simple.

    Cheers,
    Danny.

    Hi Danny,

Unfortunately we do not have grouping options for our Purchase Order Payment Tool. How about creating a category for the products, example: http://www.jotform.me/form/22551804300441?

A workaround can be seen here: http://www.jotform.com/help/178-How-to-Have-Sub-Category-or-Sub-Labels-on-an-Order-Form

Also, we'd suggest to open a new thread if you have suggestions or requests about JotForm. This way, we can easily add it to our users wish list and route it to our dev team for reviews. Thanks!
