There is already a question about this, and I have the exact problem-please read this-
As they said, I can not toggle the contact us form because it is through a third party. If I disable the form, users see "form has been disabled through jotform". I need to know how to remove the contact us tab from facebook. It was added by jotform, nothing to do with fb.
You should fully answer the other question on fb so you do not have to continue answering this question. If you look at the other question, you do not answer it-the last thing I can see is-

We apologize for inconvenience. Can you please confirm first that the Contact Us that you wanted to remove was this (seen screenshot image)
As far as I know, that is not a setting in JotForm. The setting for that should be on the Facebook page as that is an element (tab) in the your facebook page.
Please clarify what exactly was the issue and we will check again.

(either as Union or as a visitor, I do not see those tabs anywhere. See
images below.
You have to click on "more" (which I feel like people will not do so the
contact us is hidden). I would also prefer it to see "Request Information".
Do you know if that is possible?
When You click on contact us, then the form pops up-
This is what it shows when I look at tabs under settings-jotform is the
third party app they are referring to.
Currently we only want to remove it until we have everything perfect (so
technically we just want it hidden, although I don't think that is
possible).

I am not sure at this time why do you need the Contact Us page hidden. The users will not be able to navigate to the page if you have it hidden.
I don't think there is option in facebook that will allow the page tab hidden or removed.
You can change the text/name to something else by going to Manage Page / Edit Page Info and then General option.

You have the new layout, we do not.
That being said, the jotform is not finished, that is why we do not want
people to see contact us. The contact us tab did not exist until I added
the jotform to our page. There has to be a way to remove it since it was
not there before we enabled jotform.

does-jotform. I actually figured out if I want to remove jotform's tab, I
have to go into business integrations. Just an FYI for your future
reference.
The links you sent are no longer accurate because they are so old.
Because FB has changed so much-nothing that the articles say is
accurate any more (another FYI).
