The employee that registered for the JotForm account is no longer working with us and we tried to put in his credentials to the site and they do not work. I was wondering if there was a way for you to reset everything and allow me to access the forms. The form id is q_form_83015708987.Page URL:

For security reasons, we cannot grant a request such as yours. Looking at the account's information shows that the account owner is pretty much active and there are multiple forms in his account that appear to belong to different companies, yours included.
The most logical solution I can offer to you is to register a new account for your company and then copy the form from his account to yours. Note that only the form gets copied and not the submission entry database. You would have to ask the person for that database.
