I'm trying to activate a new contact form. Right after I select the form, it takes me to the screen that a popup appears asking for my email address. I enter it, but the box doens't go away, it just pops up again but empty. How do I make it go away?

There are two possible solution for this issue:
1. You have a guest account. This type of account is created when you don't sign up/ sign in and start building your forms. You should register for a free account and most likely this form will automatically move to your new account. Do get back to us if it doesn't and we will move this form to your new account.
2. Add a "Recipient Email" address in your form's notification alert. This guide should help you change/add recipient email address: http://www.jotform.com/help/41-Finding-out-E-mail-Address-used-for-submission-notifications
