Hi,
Is it possible to create a multiple choice form which would be able to calculate multiple results as you can see in this example?

I am not sure how the attribution of points is made for the different types shown in the screenshot you have shared with us, could you give us more information about this?
It looks like the multiple-choice element would be suitable for you as it offers calculation values that can help in your case, but I cannot be completely sure without knowing how the initial attribution is made.

Thanks for your reply.
Firstly, the questions would be offered as multiple choice (A,B,C,D). The choices would help calculate 4 different scores (represented by the 4 columns on display in the table). When I have tried to set this up using Jotform, I was only able to generate a single score based on the multiple selections format.
The question for me then is:
Can I attribute choices to separate scores (keep different tallys from the same questionnaire?). I'm sure I will then be able to work the calculation widget to generate the percentages that you can see in the counterpart to the example table.

Could you check if you can use an input table on the form based on this requirement?
