Fill in the blanks: Answers overlapping the questions

    When our patients fill in this fill-in portion of the form, are the answers and subtitles supposed to overlap like this? It makes it difficult to read and for our clients to fill in all of the appropriate info. Thanks for your help!

    Thanks for patiently waiting.

    I cloned your form and was able to replicate the issue. However, I was able to solve it by selecting a different theme and reverting it back to the original theme selected.

    Here's my result:

    Would it be possible for you to test the same on your end? Just choose another theme and then revert.

    I am sorry to say...

    That was the WORST advice!

    Your suggestion to fix one problem removed WEEKS of custom codes I entered into my form.

    Do you have a suggestion for me to put those codes back in place????

    Just re-entered all the codes (from the last month) into my form and IT NO LONGER WORKS.

    Please help me rectify this situation ASAP!

    It has affected the entire document.

    All the coding your company has provided over the last month has been erased due to his suggestion.

    I tried to re-insert all the codes myself but they are not all working. For example, the code for the condensed zipcode.

    Is there a way to:

    A) Revert my form to an earlier version

    B) Have someone look at how to fix the fill-in widgets in a different way

    Yes, you can revert your form to its previous version. Here's how-to-view-form-revision-history.

    Once you restored the form to the version where the original issue was, we'll check it again and forward it to our developers if needed.

    Also, I would request you to take a backup of your data before reverting to a previous version as if you have removed/added any field previously it may delete the submissions with respect to that field.
