New changes to form not printing

Hello. We use your forms for our applications at The Marshall County Animal Shelter. I recently changed a couple questions on our cat application. For some reason the with cat application, the changes show up when you fill it out but when we receive the email it has the old questions but no answers. If you look at the bottom half starting at the current pets you can see the difference.

If you're referring to your Email Notification, try to delete it and then add a fresh one, see if the issue persists.
