its not working. i added the app. i click select forms to begin. it opens a pop up and says something went wrong. is there any other way to merge multiple forms to one. or simply can i copy paste form questions from one form to another?

This happens since your account is HIPAA compliant and the app is not currently compatible with HIPAA accounts, I'm opening a feature request for this, but we cannot provide an ETA, we will however keep you updated via this ticket.
Also, I'm afraid that there is not another alternative app.

Thanks for the response. If HIPPA is the case, than can you advise how do I remove HIPPA from my account? Is it possible that I could remove HIPPA, do the the field merge and enable HIPPA again?

You can remove Hippa compliance from your account.But for a specific form only you cannot do that.
You can remove HIPPA compliance by checking Data section under this link.
