create a form that will change based on what you select in the drop down box

I am trying to create a form that will change based on what you select in the drop down box. For example, insurance quotes and the information needed changes based off of which type of quote you want to get. I dont want to have to create separate forms for each inquiry. I cant figure out how to do so, can you help?
