I can't login with Facebook

I am unable to login with my Facebook account and need to upgrade my form for more submissions.

Sorry about that. I have now reactivated your account. You can now log in again.

Your account was suspended by our system because your form was suspected to be used for phishing. Upon manually reviewing the form, I don't see anything malicious in it so I lifted the suspension immediately. The form is also added to the whitelist to prevent this from happening again. You're now good to go.
