Person having trouble with completing form

    We have a person who is having trouble completing one of our forms.

    For one thing, it looks really outdated when he pulls it up (I attached a screenshot of what it should look like, and what it looks like for him).

    Secondly, for whatever reason it's not letting him get past the first page, and he's tried so many times he's now on your phishing list, which is preventing him from using your website at all. (It almost looks like a login screen??)

    I don't really understand what's going on because we haven't had any trouble with our other students. Hopefully you can shed some light on it.

    Hello,

    Based on the screenshot, it seems that the IP address has been added to our blocked list. May we ask the IP address the student is using to verify it further on our end? He can check his IP through this link: What Is My IP Address - See Your Public Address - IPv4 & IPv6

    Please advise him to connect to a different network to see if it will make a difference? If the form is also embedded on a website, please provide us with the URL where it is located to investigate it further.
