We need a custom quiz-like form with conditions

I submitted this form and it seemed to reject my inquiry ("Sorry, too fast")and I've had to type this form again, so sorry if this is a duplicate...

We need a custom quiz-like form where mentees (students) enter form information in various fields, then after submitting the form they see a list of mentors that meet the criteria they entered.

Do you have such a form and if so can you send a link? And is this part of the free service?
