Next Button is Not Working?

I am having problems submitting the form, This issue just started happening today. If i fill out the form and then hot submit it is not submitting, it is also erasing so of the information I put in. I also notice that i cant hit the forward button if i go back to some pages.

please tell me why this started happening.
