Card format not working

It appears that the single question format is malfunctioning across several forms under my account. Please advise as users currently cannot use the form nor can I 'edit submission' entries

This issue has not been fixed as per my investigations as users still cannot press next- issue has now been resolved following clearing our cache and browsing data

We have cleared your form cache, could you please try it again? I think it was only due to cache. We also made necessary tests and everything is working including your forms.
