Help to Create a Consumer Electronics Stores Evaluation Form.

Hi. I need to create an Evaluation form for my company, which owns several Electronic Sales Stores. I am currently preparing the criteria which will be included in the forms.

Could you please advise which one of your Form Templates would be suitable for this purpose. I am looking for something creative and aesthetic, yet simple to use and with little wording.

I am attaching a simple type of form which the form needs be like.

Please provide options, which i will evaluate and provide to management. If successful we could continue with a monthly agreement.

Hi Jed. The link you provide leads to my forms, which is empty.

Also the example i provided was just to show you the form in which i need the form to be. I do not need a form for employee evaluation. I need to create a form which looks like the one i provided.

Please provide links to forms that look like the one i provided, into which i can add custom questions.

The example i provided can be created on Excel, easily; i just need something that looks like that which i can edit with my own questions. If you have any.

I would recommend cloning the form my colleague made and try to input your own questions, you should get a good feel for it then.
