My form doesn't seem to be letting my clients complete it

Hi I've had several potential clients reach out to me and say the form is freezing half way through and then there is no submission button. Please Help! This is how I generate clients

I was able to submit the form. It does freeze, and the submit button is there. Have you somehow manage to solve this? If not, do you know how to replicate the problem?

I checked the settings of the form, I see no reason it would freeze.
