I need help accessing my account

    My name is Kimmie I am the Project Manager at Moetivate Your Womb, our website is www.moetivateyourwomb.com. We use Jotform for our clients to complete their intake forms when they visit our healing center for the first time, getting a new massage, as well as agreements with our providers that use our center to connect with their clients.

    Friday I received an error message and wasn't allowed to access our account. I submitted the form as requested, and it said that my request would be reviewed and would be contacted but I have not heard anything and still wasn't able to access the forms we need in order to operate business. Can someone please get back to me on what I need to do to be able to have access to our forms again?

    Hi Kimmie, thank you for reaching us.

    It appears your account and form have been suspended due to suspected fake loan offer.

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Free accounts with non-corporate emails showing suspicious forms must be suspended immediately.

    In order to reactivate your account, kindly provide us the following information:

    1. The URL where your forms are used.

    2. Your website or proof of affiliation to a legitimate institution.

    We also need you to change your registered email to a corporate one to avoid getting flagged again by our phishing detector.

    Send these proofs to reviewers@jotform.com with Ticket #2869259 your subject. We will let you know about the status of your account after a thorough review of your proofs.

    Or, we can delete the offending forms/fields and reactivate your account.
