Account Locked?

It appears as if my account was locked. Can you please fix?

Your account got locked as you imported a form to catch login credentials, this is a clear violation to our terms of use. I just reactivated it, but I had to delete the said form.

I tried to link an alchmer form and didnt do it correctly. I already deleted it.
