Please unsuspend my account

    I had been suspended because I created a form asking for credit card information. I just created a new form that asked for only the last 4 digits of the credit card number, and that form again resulted in suspension.

    I am trying to create a form that I will send to a customer for them to permit us to share their credit card with one of our travel partners (cruise line or hotel) for them to charge the account. I am not looking to collect complete credit card information (no credit card number), only the customer's permission. The customer will provide the credit card information over the phone prior to me sending out this form, and will not be included in the form. Is this still a violation?

    I cannot use one of the CC/payment integration partners, because the organization charging the card is not us, so it is problematic getting authorization for my company since we will not be charging the card.

    I have deleted all forms previously created.

    Please advise.
