    Is there an app that will put form submissions on an image or pdf like this Elf on a Shelf Registry?

    Hi Paule,

    Unfortunately we do not have integration with Shelf Registry, however we do have PDF submission feature. You can download your form submissions directly to a PDF file right after form submissions or in your form submissions page.

    To download submission via email, just include the PDF LINK variable on your email notification. Here's how:

    This is definitively something you have to work on !!! that would be great to be able to generate automatic pdf using the form reference.

    Just to understand correctly. You would like to be able to generate a PDF version of the form once the submission to the form has been made?

    Additional to what Elton has suggested, you can also provide a copy of the submission of the form in PDF format to the user. This can be done utilizing a custom Thank you page.

    First you need to use a custom thank you page message:

    Click on the Thank You icon to obtain all the options, selecte the one displayed below:


    Then you can create an anchor (hyperlink) with the {pdf-link} field. By going to the HTML section:

    what I would like to do is like a ticketing service. You order and you get automaticly a ticket, but I would like to create a nice ticket with some information on it.

    My problem that i do not know how to insert my form answer in an external doc. Is this possible?

    Yes, it is possible to get your form answers/data into a document. You can download a PDF, Excel or CSV file of your submissions from the submissions page, to accomplish that please follow these steps:

    1. Login to your account and click My Forms on top of the page


    2. Click the form of which you need to get the submissions.

    3. Click the Submissions button from the toolbar above

    4. Then, while viewing your submissions you can download one single submissions or all the submissions you have received in the format you wish:

    You can also integrate your form with Google spreadsheet so that your submission data can be store in  aspreadsheet autmatically afte the form is submitted. You can learn more about it at the next guide:

    I work on this test form


    I would like the person to be able to receive the second part of the document with the related information filled in the form (date commande, reference commande, nom du bénéficiaire) and to be able to print it as a ticket they would be able to present when they come to do the tandem jump as proof of payment


    Sorry English is not my mother tongue and i don't know if i explain well.

    So, from my understanding what you need is that your form users can be able to download the second page of the form that I show in the image, is that correct?

    If so, what you could do is to create an auto responder that will be sent to the person who completed the form and include the pdf link just as my colleague Elton suggested in his reply. And in the body of the autoresponder email type a text explaining to your users that they need to download the pdf file and then print the second part/page of the form so that they can present it as a ticket. See the next example:

    So, making that your form user will know what he/she needs to do after downloading the pdf file.

    thanks for your help. it is actually the solution i am using, but It would have looked more professional to have all the information on the final document !

    I am trying to find a way out, but I am having a little hard time but I'm sure there really is a way for this. How about displaying the final document on the thank you page when the form is submitted?

    There's also a print button on below that page after submitting the form. I would suggest cloning that form so you'd figured out all the setup. Guide: https://www.jotform.com/help/28-Cloning-a-Form-from-a-Web-Page. Use the form URL above to clone.

    Everything is setup on the form thank you page, FYI.

    Sorry to annoy but the print button is not working? do you have an idea why?

    Hi Bea,

    I was able also to test that the Print button is not working. I believe it is because of the script restriction on the Thank You page.

    Since the print button is created to execute a script to open a print dialog/window -- the form builder have rejected and cleared the script which can cause security vulnerability on the form.

    You can however create a similar custom page and have it printed on the external page. What you need to achieved this is to redirect the form's Custom URL Thank You page to a custom page.

    Please tell us if the Custom thank you page is an alternative you can take, or not.

    Sorry there was an error on my print script. Kindly clone the form again or you can replace the print code with the following.

    You have to add this on the thank you page source, check the following guide:

    Click Setup & Embed > Thank You > Thank you Message > Appy this code on thankyou HTML source like this below.

    If you need further assistance, let me know the form URL in which you want to fix the print button so I can do it for you. Thanks!
