The non-profit I work for runs a community distribution center that distributes free condoms and other prevention supplies to other non-profits and the general public (provided they live in our state). Is it possible to use one of Jotforms online store template to create a place for people to order these items without having any form of payment requirements? We need the functionality of the store template but do not want any payment integration.

Another problem I just noticed is that the shipping address block doesn't show up on the live form even though I can see it on the back end. Why is that?

I can see you are using a PayPal Business field. This would require your users to add payment information for them to submit the form. I would suggest using the Product List field instead:
You may remove the PayPal field from the form:
then add the Product List field. Re-creating the products is not necessary as it will be carried over to the product list field.
With regards to the shipping address, it will only be available for the payment field that requires a payment. for Purchase Orders or Product List fields without payment integration, you would need to manually add a regular address field that will ask for the shipping address.
