How can I pre fill a form template

Hi,

I am making a customer satisfaction survey to send to my customers when they leave us an unsatisfactory review. Is there a way that I can create a standard form but then when someone sends an unsatisfactory review I can fill in the field with the customers name and who they were in contact with and then send the form to the customer to fill out the rest?
