    One of our members of staff is receiving the attached error message on some of our forms.

    This seems to be an isolated incident as they are able to access other jotforms and the issue only appears to be affecting one member of staff at present.

    Please could this be looked in to?

    All your forms appear to be working fine.

    Please let us know any of the forms where this issue is occurring. Does this occur when they access the form or when they submit the form?

    This looks like a temporary glitch and should not persist.

    Please check and let us know if the issue still persists.

    Sorry, but we had an issue with HIPAA accounts and it was fixed by our developers immediately.
