    When I tested your form https://form.jotformpro.com/form/52072759691969 and allowed my location, I was unable to replicate the issue (please note I have blocked out information for privacy reasons):

    This tells me that it may be a browser issue: if you look into your browser privacy settings, is the locator on? I did have to Allow the form to know my location, and I think that this might be the issue.

    Thank you for confirming!

    When accessing your form, I was able to replicate your issue.

    I did make a clone of your form 210366760567460 to investigate further.

    When inspecting the Browser Console Logs, I noticed the following:

    Upon investigating the error, this is what I got from Google:

    And was directed to this link: https://developers.google.com/maps/gmp-get-started#enable-api-sdk

    Could you please check to ensure that the Maps JavaScript API is activated as per the link instructions?

    Thank you for providing the instructions above but unfortunately, it still doesn't work.

    As the screenshot below the Maps JavaScript API is enabled and there is some requests through the API were made.

    I went through the billing process to make sure it is enabled for the API key and recheck the restrictions.

    Kindly enable your Geocoding in your Google Maps API.

    Here is a guide:https://developers.google.com/maps/documentation/javascript/geocoding

    Let us know if the issue still remains.
