I am having trouble with one of my forms

    Hi there

    I have set up a "consent form" for patients to fill out in addition to an intake form. The intake form was returned to me but the consent form I am having trouble with. Something to note is that the intake form was submitted to me by my patients while I had the free service, after upgrading to the HIPAA version is when the trouble started.

    As checked, there is no submission on this form yet: Beautifully Well Toxin Consent, so technically, you won't be receiving any email notifications yet, if that's what you're referring to.

    Also, I can see that you have set the form as PRIVATE so only the users that you INVITED VIA EMAIL can view and submit it while logged on to their own Jotform account:


    Now, if you want this form to be accessible by the public, please set it to PUBLIC:


    This will no longer require the person accessing your form to create or log in to their Jotform account just to view or submit it.
