Attached PDF in email is missing information that shows up in the text body of the email after the form has been submitted. Please help.
The form in question is Client/Patient Information. The questions related to the pet is missing on the attached pdf in the email after submission yet the answers show up within the text body of the email. Example: "Patient Information" heading shows up but the answered prompts for "Pet name" "Breed" "Sex" etc does not appear in the pdf version attached to the email. What can we do?

Please try creating a new PDF document that should pull the updated fields in your form. Let us know if that helps or not.

No, I did not create a pdf under the pdf editor. My understanding was a form that was created in the "Form Builder" and submitted automatically generated a pdf version of the respective form that is attached to the email notifying a submission has been received. Is my understanding correct?

Hi clhalluga, thank you for reaching us.
Your understanding is correct. However, you will need to update your PDF document too.
I have cloned your form and was able to recreate the issue.
To fix this, you will need to go to your PDF Editor, or kindly click this link.
Then, please delete the current New Document.
Afterward, please add a New PDF and click on New PDF Document.
The PATIENT INFORMATION should be displayed properly
