Did not change password

    I could not find any account linked to your email address.

    But there is a HIPAA account linked to the last form https://www.jotform.com/91138041536149 that you checked out.

    What exact issue are you facing? Do you own that HIPAA account?

    If yes, are you not able to log into that account or are you not able to reset the password of that acount?
