Hi,
Is the issue only happening when you go directly to Jotform.com?
Are you able to access your forms and submissions page?

Yes, only when I go directly to Jotform.com. I am not able to access my forms and submissions page, although I can access forms that I have bookmarked on my web-browser, but I cannot access them once I submit a new form.

We apologize for inconvenience. I have been checking your account/username Kelsey_Hoff and it is active and forms are available.
Ex:
Because your account is restricted (HIPAA) we cannot check the submission data. But overall, there was nothing wrong with the account as it is in good standings and no exceeded usage limits as well.
Perhaps the issue was in the internet/network you were using.
Have you tried checking using a different network/internet? Please try checking also using your mobile device perhaps. Try different internet/network.
Please let us know if issue persist.
