Juror Questionnaire Form

    I am looking at an option for selected Jurors to fill out a required questionnaire online. I would need to have the Juror enter their given juror number and date of birth to verify before the form questionnaire starts.

    I am wondering if JotForm has the ability to import this data and have this as a feature. I will also need to have the form information submitted and have us be able to print out the questionnaire form with the juror's information after we receive the completed information. The forms need to be on file in our office for attorneys to obtain if requested.

    Second part of the project is the same jurors ,if needed, complete another form request to be excused from jury duty.

    Can you please tell us also what is the data source you intend to import the data from.

    Was it in Excel or CSV?

    It is possible to use Excel / CSV as data source for the form using the Spreadsheet to Form widget. Please review the user guide : How to Use the Spreadsheet to Form Widget

    You can use your data and import it in the form to use it as the data source.

    I noted that you were using the Smart PDF Form feature.

    You can use the PDF report to print the questionnaire form with data/information on it.

    I am new to Jot Forms.

    When I uploaded the form , the fillable question format is a mess it doesn't reference the Form layout. The yes/no responses are all under one question instead of with each question the yes/no is supposed to be with. How do I move the yes/no responses to the correct spot? The form input is in sections which are not correct?


    Please let me know how to modify the input form, as I do not want to create a new form as the actual form look correct it is just the input question that is not connecting to the form .

    We would like to apologize for any inconvenience. Please know that you can still add/remove fields on the Online Form version of your form to match the fields on the Original Form that you uploaded.
