We have a project at an Airport. We are doing Covid testing for employees and then general public. Right now, they are using a company that used Jotform to collect pre-regristations. We need to be able to allow people to:

Pre register within designated times. 3 times a week within a 2 hour window

check off a consent form

take that information and download or upload it to integrate it into a Lab Information system so that the lab can pre-print a label for the persons' testing tube. Bottom lineis the the information from Jot form has to get into the Lab' software program

Also, regarding the requirements you mentioned, I cannot recommend using an APPOINTMENT field for that. This is because the user cannot make 3 concurrent selections into the appointment field's slots. It has to be one slot per submission only - requiring the user to submit the form thrice.

I'm not sure what software are you using with your Lab Info System, but basically, you can manually download the submissions into an Excel file. Here's how-to-export-form-data-to-excel.
