    I have checked your account and it seems to be suspended because of your form https://www.jotform.com/form/210331054055844 that collects login and credit card information which violates our terms of use.

    In order to lift the suspension in your account, we would have to delete the guilty fields in your form.

    If you want to collect payments in your form, you may use our payment integration.

    Yes delete them please, I made it as a prototype to show my company. We use stripe for payments.

    Unfortunately, it is not possible to collect such information in the form.

    Any fields that collects password will not be permitted.

    Regarding the password prompt, is it a verification question?

    You would have to remove the label/word "security and password" to be able to add the field in your form.

    Just remove it all and I'll figure it out from there.
    Again, it's a prototype that I want to show the CEO as I'm pitching it as a new solution.
    She tried to access it today and that's when we discovered it wasn't working.
    Whatever you need to do to get it eunn Ng Asap just do it.
