I have requested a password reset email several times and I have not gotten one yet.

Hello,
As per your request, I have sent password reset mail to your email address. Please make sure you have also checked your spam folder. Please send us a screenshot of the issue. If you do not know how to send a screenshot to the support forum, please visit this guide.
If you did not receive an email from JotForm, you can also try to reset it by yourself.
To reset your password;
1- On the JotForm website, click on the Login button
2- Click on the Forgot Password button under sign in section
3- Write down your email address or username and click the Send Reset Instructions
You will receive a password reset email from JotForm. You can follow and reset your password from that email.
