HIPAA compliance with the PHI function and emails

We are using a form for our COVID vaccine sign up. The information that is sent after submission is all hidden if the question is marked PHI, but it is not very useful for signing people up if we don't get that info in the email. Is it certain that if we uncheck the PHI button so that the info comes over in the confirmation email--that it is NOT HIPAA compliant? If so, then do you have a recommendation for how to best get the form info to another person? I am the guy who creates the forms, but the form submission data should go to someone else.

    Thank you in advance.

    Hello,

By disabling PHI button you let the users see the data via email.

It should be looked like this:
