Hi,
I have attempted to access my forms but our most used Covid 19 form with responses has disappeared?

Hi, thank you for contacting JotForm support.
Due to HIPAA restrictions, we are not able to login to your account and check your forms. Do you mean that all your form submissions were disappeared? Have you perhaps deleted the submissions? If not, please let us know so we can escalate the issue to our developers.

Yes, my form has disappeared, however when I encountered this problem
yesterday when I clicked on previous form(at bottom of page) that people
have responded to(ie view all submissions) I was able to access the page
with the form on it from that workaround. Today I do not see the form *Mark
P Feinberg DMD Covid 19 Questionnaire(MedPro) Updated 7.20*
Again, if I click on this link https://hipaa.jotform.com/myforms/
I see my other forms but not my Covid 19 form with all the
submissions. It isn't there.
I can provide my login if that is necessary.

I would suggest you to please share the form URL or web page URL where you have embedded your form, and we will take a look.

I am glad to know that you figured it out and recovered your forms. If the forms were not mistakenly deleted by your staff, I would suggest you to please change your account password to ensure no one has unauthorized access to your account.
