Hello gmssyedbasti,
To create a new form,
- Login JotForm account and click the Create Form button.
- Choose from 3 options. Option 1 is to create a new blank form. Option 2 is a list of sample form templates. Option 3 is an import form from the URL or clone your older form or import a pdf and convert PDF form to online form.
- If you choose Option 1, then you must choose Classic form or Card Form.
- If you choose Option 2, then you must choose a form template.
- If you choose Option 3, then you must choose import form from URL or clone your older form or import a pdf and convert pdf form to online form.
- You can add new form elements by click in on the Add Form Element button (1). You can manage Build, Settings, and Publish operations from the area marked (2). The Form Designer button is marked on number (3).

Sign into your Create Account.
Snap "Content" from the top menu.
Snap "Add/Edit/Delete Forms" from the left hand menu.
Snap "Make New Form".
Type the name of your structure in the title box (this will show up over the structure on your site page).
