HIPAA forms are not working

    I work at a school for children with autism, and we created a Daily Health Screening form in Jotform that our families submit every morning before their children come to school. This morning, about half of our families were unable to access and/or submit the form. They received numerous error messages including:

    “502 Bad Gateway” "Incomplete Values" AND/OR when they attempted to access the form, the site would not load

    A similar issue happened yesterday, when we attempted to gather vaccination registration information from our staff via JotForm.

    It is critical that our forms work each morning, as the inconvenience to more than 100 families of children with autism is not acceptable.

    Please help me to resolve this issue, so it does not happen again tomorrow morning.

    We do apologize for the inconvenience.

    We had an issue with our data connection but is fixed by our developers.

    Can you please test your form again and let us know if the issue still remains?
