My form calculations disappeared and revision history will not restore

A few moments ago I was in the middle of editing my form when a "oppps" error came up. Suddenly my form reverts to a whole page instead of the slides and all of my conditions data is gone...that is over 100 conditions. Revision history restore doesn't work. I am freaking out --this was weeks of work. Can you help me?

Are you referring to a different form or has the issue been fixed?

We have had a few issues with the HIPAA server lately, that are now fixed.

I have cleared your form cache, and would suggest clearing your browser cache just in case as well.

It could be a temporary issue. Are you still seeing this issue in your form? I did test your form, and it seems to load just fine.
