My survey form is not working properly when using an iphone

This is absolutley the most annoying thing in the world. Noone can even see the form, half of the questions are missing. I paid for a full year and this is what I get.

I deleted all of my forms and canx my account. I had to redo the whole form with another company and completely created a lot of emails and messages from followers who were not super excited about the experience.
