HIPAA account: The user received an empty submission.

    Hello,

    I have two issues to discuss 1)

    Several clients have complained that when they try to upload documents, they get an "upload failure, please try to upload again" after attempting to submit the form. The documents show that they complete the upload process before they press submit. It is not a specific document type as it occurs with both PDFs and JPG/PNG.
    2)One of the forms had a submission today that was completely blank. That should not even be possible because we have required fields in our form. It even triggered our autoresponder notification and created a folder through our google drive integration.
