• YouTube widget https://www.jotform.com/widgets/youtube
• Another option would be using iframe embed widget https://www.jotform.com/widgets/iframe-embed

You can upload any video format on the hosting platform that you'll be using.

Sorry just a few more questions.

1. We will also create a few more of these forms which will be similar, so we will use the clone tool.
   But
   how do we create a unique (custom) URL for each form? We don't want to see the word JotForm anywhere.
2. We will probably upgrade to the Silver package but really wont start until April 2021.
   Can we sign up now for the Silver but start to pay in April? Or do we have to Start as soon as we pay?
3. What happens if we have more than 1,000 payment submissions on the Silver package (which may happen), does it automatically go up to the Gold package and then revert back to Silver?
4. With the payment gateway we use Square. When using this, are there any additional costs?
