I can not find my forms

I can not find my forms. Kujiweza, Inc. It is for the square and all payments received have disappeared. Can someone help me? my email: mwezokudumu@yahoo.com

I see there are two accounts one with the username kujiweza and another with the email address mwezokudumu@yahoo.com with the username Mwezo10.

I see a couple of forms in the account Mwezo10. You may reset the password to this account and then log in to this account.

I can not find the other account. There is no place to put in the user
name. https://www.jotform.com/myforms/ this is the only URL that I can
get.. How would I verify forum+2887141@jotform.com. Would this be our
square account?

Sorry just finally figured out how to respond to you. My other replies went somewhere. The only URL i can get is jotform.com/myforms. Where to I find the place to put in the user name: Mwezo10. Right now all I can get is the forms page.
