Contact form header image question

Hi, is there a way to add the attached header image to the nwsdistributing.com Jotform contact page without the the black backgound behind the header image?
I also tried the "add your logo" upload to try to remove the black background behind the header image but most of the jpg was cut off in the preview page for phone or ipad.
So I deleted the jpg and when I tried to "add header" that area was added at the bottom of the contact form page.

Also is there a way that I can add text to the bottom of the contact form page - a disclaimer that any contact form information that is sent to us is not shared or sold ...........

You can try adding the Paragraph element before the Submit button.
