    Hello,

Not sure why this is happening but it is truly bad timing as I have a form in my funnel that needs to be utilized by customers.

I did try and make a form by importing it from my existing site, and this is when all the problems started.

Is there a way I can get my account back I need this asap.

    Regards,

    Enrico

    Thank you.
Another note is that when I went to import the form from my existing
website. It was a protected form meaning me the administer had to login to
    allow access to the form.
But I’m doing this jotform brought in the login screen as the form which I
    think is the problem here.
I have no intentions of phishing I would never do that and I apologize for
    my misstep in doing this.
Thank you and I hope this makes sense,
    Enrico

The [Imported Form] Login - Conscious Strength form (https://form.jotform.com/210406062470140) was found on your account (Cenrico) as suspicious of phishing activity because you're collecting user credentials.

The form is in your account's Trash folder.

Gathering this type of data violates our Terms of Use.

Asking for an access code to conditionally show/hide fields within your form is okay. However, you should refrain from using "password" as your label to avoid being flagged by our phishing detector. Kindly change it to something similar, like an "access code". Other than that, no other kinds of passwords or user credentials are allowed to be collected.

We’ve removed the offending field on your form and lifted the suspension on your account. Please refrain from making new forms that are against our terms to avoid this kind of inconvenience.

    Mike_G,
Thank you so much. How this happened was, I used the importer for an
existing form on my website. I forgot that the form I was trying to
recreate was protected by membership setting. So when Jotform went to grab
it, jotform was presented with a login screen to access the form, because
again the form was behind membership settings I had setup on my website.
Jotform obviously didn’t have membership credentials so it saw a login
screen and made a form, from the login screen.
I do understand what you’re saying and would never do or ask for these
things. I will never use the importer again since this is where it all
    started.
Thanks for lifting the suspension this will never happen again,
    Enrico
