We would like to apologize for any inconvenience. May we just ask that you give us more information about the form shown in your screenshot, please? May we ask for the username of the account that the form in your screenshot is under? The issue you are having could still be related to the issue on forms under HIPAA compliant accounts that we recently reported to our developers. It seems that the issue has not been fully fixed yet. We will be waiting for the information about the form(s) you are having issues with so we may forward it to them. We will wait for your response.
