Hi, we have a Learning Management System (LMS) to teach an online health course and just started using JotForms to collect HIPAA compliant information from clients in our Intake Form before they’re automatically enrolled in our program. Every week we are required to collect HIPAA information from them inside our online course. Our goal for collecting this information is to embed multiple JotForms inside our LMS (one form for each week) to collect this information. We are trying to come up with a solution which will identify each participants weekly submissions with their original form submissions in the Intake Form.
We are looking for a way that will identify the user without them having to put some identifying information (e.g. email, name, etc.). The problem with users putting their own email or name into a field every week is it gets cumbersome to the user and it is prone to user error. So we are looking for a way to identify their submissions across all weeks that can be identified without the users inputs. Can there be some way to use a hidden field, an outside tracking system integrated with JotForm, integrating forms together, a form login option, or some other solution to accomplish this?
If this is not possible through a setting or add-on app, do you all offer any custom solutions or work with any partners who could help us accomplish something with the JotForm API?

Hi Nathan,
From your requirement, it looks like you'll have the form users' information already. In this case, I would suggest that you use our "spreadsheet to form widget". You'll be able to connect an Excel file with your form that will already have all the details that your users have to enter every time they use the form. They could simple use a code and the information will be automatically populated into the fields.
